If you operate an Art Glass Business, Gift Shop, On Line Store or the like, you may be eligible to establish a Wholesale Account with Sunshine Glassworks Ltd.. We establish accounts using an application process. You can download the application as a PDF file by CLICKING HERE.
We require the following materials to process your application:
= Tax collection certificate and/or business license
= New York State applicants must supply form ST-120 exemption form
= Business card/letter head and Voided Business Check
= Promotional Material such as web site, brochures, display ads
= An opening order of $650 will be required of new applicants. If you have already established a wholesale account at a recognized art glass distributor, a copy of a recent wholesale invoice will waive the opening order.
