Part 2: Business Documentation

We require photocopies of the following items to demonstrate the establishment of a business. All documentation must be in the name of the business using the address shown above. Please send one item from EACH SECTION and check the applicable box on this form.

Promotional Materials
Please send at least one item below. Promotional materials must advertise an art glass business or closely related activity. A cabinet maker would be considered closely related. A dog grooming business would not.

Yellow Page Ad (If your Yellow Page ad is coming out in the next Phone Book, we will accept a copy of your ad contract from the publisher)

Promotional Literature. This can be a brochure you hand to prospective clients or your display ad from your local paper (Classified ads are NOT acceptable).

Part 3: Opening Order:

Upon acceptance of the materials in Part 2, Sunshine will mail the complete Wholesale Literature Package which includes Catalog, Confidential Wholesale Price List, Updates and Sale Flyers. All Applicants must make an Opening Order of at least $650. We will waive this order upon receipt of one of the following items:

Photo of your Store Front Location. Photo must show sign with name of company and location in a business district. Sorry, but a photo of your home, regardless of signage, will not exempt you from the Opening Order

Photocopy of a RECENT invoice from another Wholesale Distributor that follows AGA requirements. Invoices from Wholesalers that require only a business card and tax number will not be considered.
